Built-in assumptions:Monthly model with a 28-day FHO+ hour-cap assumption; 20 regular clinic days; $175 annual capitation per patient; $80 hourly rate; $44.55 base visit values; 30% shadow billing; 50% Q012 premium; visit-based regular out-of-basket modelling; and 0% overhead.

Monthly results are annualized by multiplying the monthly estimate by 12.
